Welcome to Freeman Consulting, Inc. This document contains important information about my professional services and business policies. When you sign this document, it will also represent an agreement between and Freeman Consulting, Inc.

EXECUTIVE COACHING SERVICES

Services

Executive Coaching is a partnership focused on developing the client’s emotional awareness, critical thinking, and prudent decision making to help the client identify and achieve their goals in communication, leadership, time management, and emotional intelligence. The success of the executive coaching relationship depends upon the Client’s commitment to the process.

Confidentiality

Freeman Consulting, Inc. fully protects the privacy and confidentiality of the communications with the Client. At no time will Freeman Consulting, Inc. voluntarily divulge the coaching relationship between themselves and the Client without written permission from the Client.

Fees

Executive Coaching sessions are $350 per 45-50 minute session or a monthly check-in for 12 months for $3600.

Cancellations

I ask that you give one full business day notice if you need to cancel or change the time of appointment. For example, notice for a cancellation of a Thursday appointment would be by the end of the business day on Tuesday. I will make every effort as well, to re-schedule sessions which are cancelled in a timely manner. Cancellation (less than one full business day in advance) and/or no‐show appointments are billed to the client for the full amount.

Limitation of Liability

IN NO EVENT SHALL Freeman Consulting, Inc. BE LIABLE TO CLIENT FOR ANY SPECIAL, PUNITIVE, INCIDENTAL, INDIRECT, OR CONSEQUENTIAL DAMAGES OF ANY KIND IN CONNECTION WITH THIS AGREEMENT.

Entire Agreement

This Contract constitutes the entire agreement between the Client and the Consultant and the
Consultant shall not be held responsible and shall remain blameless for any adverse or other
consequences, including but not limited to financial, personal, employment or other losses,
arising out of decisions or actions the Client may make as a result of the Services provided by
the Consultant.
