Counseling Agreement


Welcome to Freeman Consulting, Inc. This document contains important information about my professional services and business policies. When you sign this document, it will also represent an agreement between and Freeman Consulting, Inc.

COUNSELING SERVICES

Since counseling often involves discussing unpleasant situations, you may, at times, experience uncomfortable feelings like sadness, guilt, anger, frustration, or loneliness. Despite this, research shows that most people who persevere through these feelings experience benefits such as better relationships, improved life skills, solutions to specific problems, and significant reductions in feeling distress. If you have any concerns about how counseling is impacting your emotional life or functioning, please inform me so that I may appropriately support you and direct your sessions accordingly. Your first session or two will involve an evaluation of your situation. Then I will be able to offer more specific impressions of what our work might include. If you have questions or concerns about our procedures, we can discuss them whenever they arise.
Counseling requires active effort on your part. In order to be most successful, you will have to work on things we discuss outside of sessions.

Freeman Consulting, Inc.’s services are not suited for individuals suffering from any problems that could endanger their life or health or the life of others. If you are suicidal, homicidal, or struggling with acute addictions that impair your daily functioning, serious eating disorders, or other conditions that could jeopardize your life or the life of others, please immediately seek local, emergency clinical mental health services immediately in lieu of continuing to work with me. Likewise, if the above problems become apparent at any time during counseling, I will help you find appropriate, local, clinical mental health resources.

APPOINTMENTS

Appointments will ordinarily be weekly sessions of 45-50 minutes. If you need to cancel or reschedule a session, I ask that you provide me with one full business day, for example, if your session is on Wednesday, then I ask you to notify me by Monday night. If you miss a session without canceling, or cancel with less than one business day notice, my policy is to collect the full amount of the session. If it is possible, I will try to find another time to reschedule the appointment. In addition, you are responsible for coming to your session on time; if you are late, your appointment will still need to end on time.

PROFESSIONAL FEES

The standard fee for the initial intake is $150.00 and each subsequent session is $150.00.  You are responsible for paying at the time of your session unless prior arrangements have been made. Payment must be made via Venmo, Paypal, Check, or cash. All sessions are self-pay. I do not accept insurance or fill out insurance forms.

Any attempts to seek reimbursement from a third-party payer must be undertaken by your initiative, and any determination of benefit is solely between you and your carrier. When requested, I can provide a statement of services, which you may choose to submit to your insurance company for reimbursement.

PARENTS & MINORS

While privacy in counseling is crucial to successful progress, parental involvement can also be essential. It is my policy not to provide counseling to a child under age 13 unless s/he agrees that I can share whatever information I consider necessary with a parent. For children 14 and older, I request an agreement between the client and the parents allowing me to share general information about progress and attendance. All other communication will require the child’s agreement, unless I feel there is a safety concern, in which case I will make every effort to notify the child of my intention to disclose information ahead of time and make every effort to handle any objections that are raised.

CONFIDENTIALITY

Freeman Consulting, Inc. fully protects the privacy and confidentiality of the communications with the Client. At no time will Freeman Consulting, Inc. voluntarily divulge information, unless required to do so by law.

CONTACTING ME

I am often not immediately available by telephone. I do not answer my phone when I am with clients or otherwise unavailable. Email is the best way to communicate with me between sessions. Otherwise, you may leave a message on my voice mail and your call will be returned as soon as possible. It may take a day or two for non-urgent matters, especially over the weekends or other planned absences. I will make every attempt to inform you in advance of planned absences.

In the event of a life-threatening emergency of any kind, please call 911, your family physician, or go to the nearest emergency room.
