Nutrition Consulting Agreement

Contract between Freeman Consulting, Inc., Alexa Roe, and , known as the “Client”. The Services contracted shall be that of professional consulting, as set out below.

Services

This partnership is focused on developing the client’s knowledge of health as it relates to food, dietary supplements, and behaviors associated with eating. Information obtained and any guidance received about health factors are within the client’s control (diet, nutrition, and related behaviors) in order to nourish and support his/her health and wellness. Please note that this partnership is not a substitute for the diagnosis, treatment, or care of disease by a medical provider.

Nutrition evaluation or testing provided is not intended for the diagnosis of disease. Rather are intended as a guide to developing an appropriate health-supportive program and monitor progress for the client.

Confidentiality

I understand that Freeman Consulting, Inc. and Alexa Roe fully protects the privacy and confidentiality of the communications with the Client. Notes from sessions will be kept in a secure location as a record of our work together which may include topics we talk about, interventions used, and treatment plan or any other considerations that may be helpful to our work. At no time will Alexa or Freeman Consulting, Inc. voluntarily divulge the consulting relationship between themselves and the Client without written permission from the Client, unless required to do so by law.

Fees

At the initial session, the client will be charged at the rate of $750 for a bundle of 5 sessions. Each session is a regular 45/50-minute session charged at the rate of $150 per session.

Cancellations

I ask that you give one full business day notice if you need to cancel or change the time of appointment. For example, notice for a cancellation of a Thursday appointment would be by the end of the business day on Tuesday. I will make every effort as well, to re-schedule sessions which are cancelled in a timely manner. Cancellation (less than one full business day in advance) and/or no‐show appointments are billed to the client for the full amount.

Limitation of Liability

IN NO EVENT SHALL Freeman Consulting, Inc. and Alexa Roe BE LIABLE TO CLIENT FOR ANY SPECIAL, PUNITIVE, INCIDENTAL, INDIRECT, OR CONSEQUENTIAL DAMAGES OF ANY KIND IN CONNECTION WITH THIS AGREEMENT.

I hereby release and discharge, indemnify, and agree to hold Freeman Consulting, Inc. and Alexa Roe harmless from all claims, demands, costs and expenses, and causes of action, either in law or equity arising out of or in any way connected to our work/partnership together. This is a contract between myself and Alexa Roe, and I understand the terms of this fully and voluntarily agree to release any potential liability.

Entire Agreement

This Contract constitutes the entire agreement between the Client and Freeman Consulting, Inc. and Alexa Roe. Freeman Consulting, Inc. and Alexa Roe shall not be held responsible and shall remain blameless for any adverse or other consequences, including but not limited to financial, personal, employment or other losses, arising out of decisions or actions the Client may make as a result of the Services provided by the Consultant.
